Medical Requirements

Your child must have proof of a physical exam, including immunizations, upon admission to the center. All immunizations must be up-to date at the time of enrollment. A copy of your child's physical exam and health records will be kept on file and reviewed periodically. It is your responsibility to notify the center when your child receives new immunizations and to furnish proof of them to the center.

A current immunization record must remain on file at the center for continued enrollment. If an immunization is not given in a timely manner or cannot be given, a note from the physician will be required to remain in the program. An immunization schedule is available from the Executive Director.
